If you are planning to pay for your items by credit card, check or money order, you do not need to fill out an account application. We will use your information provide at check out to establish your account.

Instructions and Terms for Credit

We accept purchase orders with NET 30 terms, placed by D&B-rated U.S. based businesses, non-profit organizations, U.S. educational institutions and U.S. federal and state government agencies.

You can view and print our New Application Form here directly, or go to our download forms page and download our New Account Application Form in PDF format. Print it, fill it out and FAX it back to us at 1-920-746-2185.

In some cases, we may require you to fill out a credit application which can also be viewed and printed here, or you can visit our download forms page and download the file in PDF format.

Purchase Order Net 30 Terms:

Customers are invoiced on the date of shipment, and payment is due within 30 days from date of shipping. In case of late payment, a 1.5% monthly interest charge is added to the unpaid balance.

Payment:

Payment for Net 30 Accounts will be by Check or Money Order in US Funds. A $25.00 fee will be charged for bounced checks.

To Order by Purchase Order

1.) Place your order using the Purchase Order payment option at checkout. Print this out for your records and purchase order.

2.) Fax a copy of your purchase order to 1-920-746-2185.

3.) The purchase order will then be verified and we can proceed with your order.
